Could you please elaborate, how exactly are you inserting the attachment? Are you using Tab to switch from a field to the button?
Since the fields support pasting images, you can of course paste an image into the step's expected result without any problem. But in order to attach a file to the step itself, the button should be clicked.
If you could share more info what exactly is happening and how you are getting where you are getting, that would be appreciated. So far this looks like expected behavior.

I understand the feature to paste image into the textbox, that's cool. But it's not what I intended to do, since I clicked the icon of the step.
Answering your question: I'm clicking the icon to add the image.
For more information: there would be an extra screenshot of the image being loaded, this happens after I click the button and before the image is added to the textbox.

May I also ask you, which browser and OS this behavior is experienced on? As far as I can see, you are also using the Night appearance theme, but it looks somewhat different from what it should normally look like.
Also, is this behavior persistent and happens every time you attempt to upload? If you try to replicate this in your browser's incognito mode, will it be replicated?

Ilya Volkov: I'm using Windows 10 on Mozilla Firefox 96. I use an extension for the Night theme. I've tried in Incognito mode and could replicate it.
I logged in Google Chrome 97 and it worked as expected, so it seems to be an issue with the Firefox browser.

Gabriel Paiva: Most likely, the problem is caused by an external dark mode extension you are using - we can't guarantee that it's compatible with Qase components.
I'd recommend using our own Night theme which can be enabled at https://app.qase.io/user/appearance
Can you give it a try and let me know if with this night theme (instead of the one forced by an extension) it will work as expected?
