Add an intuitive flow to remove cases from a Test plan.

Currently, to remove existing test cases from a Test plan. A user will have to select the 'edit plan' option and de-select test cases.
This flow is unintuitive for two reasons:
- It doesn't occur to a user this option allows removal of cases from a plan. At least, it's not the first thing that comes to mind.
- It's unnecessarily hard to have to search find and de-select test cases to remove.
There should be simple 'remove' button along with the current assignment options we have in the plans.
Given the existence of multi-select for cases in the plan, the remove button would be a nice addition.
